File a Complaint

In Arizona, if a patient or pharmacy feels that an insurance company or their pharmacy benefit manager has violated state law, they can file a complaint with the Arizona Department of Insurance. 

You Will Need:

  • Insurance Information

  • Any applicable documents or records of the violation (claims, emails, letters, etc.)

  • Any other data or information that helps prove the violation

The more legitimate, fact-based complaints the state's DOI receives, the more likely it is that they will officially Investigate and Enforce state law.
